Surgical Outcomes for Upper Lumbar Disc Herniation

Abstract
Study Design: Retrospective study.

Objectives: This study aimed to identify factors affecting the surgical outcomes of upper lumbar disc herniation (ULDH).
Summary of Literature Review: In patients with ULDH, the clinical symptoms are diverse due to anatomical complexity, and surgical access is difficult in many cases.

Materials and Methods: In total, 60 patients who underwent operative treatment for ULDH between 2019 and 2021 were included.
They were followed up postoperatively at different periods with the Oswestry Disability Index (ODI). The data were analyzed using analysis of variance and the t-test. Multiple regression analysis was also performed to analyze which factors significantly affected the amount of ODI score variance. A P-value <0.05 was considered to indicate statistical significance.

Results: ODI scores improved postoperatively for all patients except one. Male patients showed a significantly greater improvement in ODI scores than female patients (p<0.05). Patients whose symptom duration was between 0 and 3 months showed more improvement in ODI scores than those with longer symptom duration (p<0.001). No significant difference in ODI scores was found according to surgical methods (endoscopic or microscopic) (p=0.772). A positive femoral nerve stretching test was least apparent in L3-4 compared to other upper lumbar levels.

Conclusions: Our analysis reveals that surgical treatment is effective for treating patients with ULDH. Both endoscopic and microscopic discectomy showed similar surgical outcomes. Patients whose symptom duration was less than 3 months benefited most from surgical management. We believe that early surgical intervention may be a key factor for obtaining good outcomes in patients with ULDH.
